		<title>Update on Gramps&#8217; Condition</title>

<p>Since Gramps, at 97-years old, is now in an assisted-living facility, I don&#8217;t have daily interaction with him, and hence, no stories. </p>
<p>However, I realize that the story doesn’t stop here.  As my friends’ parents and grandparents age, more and more are being affected by Alzheimer’s.</p>
<p>So today I decided to share what I have learned on this journey.   I have noted which Stage of Alzheimer’s I believe my grandfather was at for each of these lessons to help readers anticipate what they are going to encounter.</p>
<p>You can see all the stages <a title="Alzheimer's Stages" href="http://www.alz.org/alzheimers_disease_stages_of_alzheimers.asp#stage5" target="_blank">here</a>.  My mother and I consulted a family counselor, who specializes in helping care takers, along the way, and I must say, her wisdom and advice was priceless and instrumental in allowing us to provide the appropriate care at each stage and keep our sanity in the process.</p>
<p><strong>1)     </strong><strong>Pick Your Battles</strong></p>
<p>As an Alzheimer’s patient loses their cognition, they are going to do things that we think are “crazy.”  (Stage 3) Hence, how my blog started.  They can be funny, like when Gramps put my dog’s food in the freezer and my mother couldn’t find it.  They can be annoying, like when Gramps would put up a fight when we would take him to his day care program.  We knew it was the right thing for him, but he wasn’t in a position to understand our rationale for it.  They can be frustrating, like when Gramps would hide our business mail in his desk drawer.  But as a caretaker, you are going to exhaust yourself trying to fix everything that the patient does incorrectly.  How many times can you take the cereal out of the refrigerator before acknowledging that the patient is going to keep putting it there?  And does cold cereal really matter? </p>
<p>Getting angry at the patient, or trying to show them the correct action is going to be fruitless.   It is not like a child, who, as they grow and are disciplined enough will hopefully learn their lesson.  An Alzheimer’s patient doesn’t have the cognition to determine right from wrong. </p>
<p>In the scheme of things, does it really matter that I found Gramps having a beer with breakfast on more than one occasion?  That he would hide our office mail in a drawer?  That he would empty the recycling bin at work when it had just one piece of paper in it?  No, it doesn’t. </p>
<p>The most important thing is the Gramps wasn’t doing anything to harm himself or anyone else, and that we kept him safe, which leads me to my second lesson. </p>
<p><strong>2)     </strong><strong>How Should I Care for My Loved One?  </strong></p>
<p>This is probably the biggest decision a family needs to make, and the most difficult.</p>
<p>Since my grandfather’s downhill slide began when he was hospitalized for a minor fall, which left him a little bruised and unsteady on his feet, I was adamant that he be moved as little as possible, and kept in familiar surroundings.  Our counselor seconded this opinion, and advised us that we should keep my grandfather at home as long as he recognized he was in fact home.  This was probably the best advice I received during this journey.  Thankfully, because my grandfather had built a successful business, we had the financial resources to care for him as we saw fit, without financial limitations.</p>
<p>Initially this involved my mother moving in with him to watch after him at night, while I took him to the office with me during the day. </p>
<p>Eventually, this became too much for my mother, his primary caretaker, as she wasn’t able to get a good night’s sleep.  At that point, we hired a nighttime caretaker, who stayed with my grandfather from 11 p.m. – 7 a.m. He made sure my grandfather had someone to help him in the middle of the night should, and was there to bathe and dress my grandfather in the morning. </p>
<p>My grandfather would be alone for a couple hours in between the time his caretaker left and I picked him up in the morning to go to work. </p>
<p>One day I found my grandfather wandering around outside as he had locked himself out of the house.  Luckily, he was unharmed, but it was at this point that we decided he needed 24 hour care. </p>
<p>Again, since my grandfather was aware of his surroundings most of the time and knew where his home was, we decided it was best to keep him at home in familiar surroundings.  Again, we were blessed to have the financial resources to do this. </p>
<p>Eventually, as my grandfather lost all recognition of any place, person or thing, we put him in an assisted-living facility that specializes in caring for Alzheimer’s patients.  We decided it wasn’t worth the cost of having private around the clock care to keep him at home, when he didn’t even recognize he was home.  The assisted living facility is about a third of the cost of keeping him at home.  I believe this was the best plan of action for our family, my grandfather and our financial resources. </p>
<p><strong>3)     </strong><strong>The Care Taker Needs More Care Than the Patient</strong></p>
<p>An Alzheimer’s patient progresses, they become less and less aware that they have the disease.  Someone in the first three stages may acknowledge that they are getting forgetful, but as they progress, they don’t have the cognition to understand their brains aren’t working at full capacity.  Also, the one good thing about Alzheimer’s is that the patient doesn’t experience any pain.  As long as their basic needs are met, the patients are going to be indifferent to their situation because they don’t know any better.</p>
<p>So the hard part of the disease falls on the caretaker. </p>
<p>The caretaker has to suddenly fit someone else’s needs into their life. If a family member is the primary caretaker, they need to adjust their lives. This may mean not getting as much sleep as they once did, not being able to be as social as they may like, not being able to travel as much.  This alone is a huge change to one’s lifestyle.</p>
<p>Secondly, the emotional burden of having to make decisions that severely affect another person’s life is difficult.  All of a sudden, my family, who was so accustomed to being cared for by my grandfather, physically, emotionally and financially, found the roles reversed.  Not to mention, as in most families, there are differing points of view on how my grandfather should be best cared for. Ultimately, however, the decision falls to one person.</p>
<p>If you are the primary caregiver to an Alzheimer’s patient, remember to take time to yourself and ask for help when you need it.  Try to be aware that you are reaching your limit before you reach it because stress and anxiety isn’t good for you or the Alzheimer’s patient.  And most importantly, know when you can no longer safely care for the patient and at that point, move on to the next level of care such as a live-in care taker or an assisted-living facility.</p>
<p>It is difficult for me to see my grandfather as he is now: confined to a wheel chair, unable to perform everyday tasks, such as dressing and eating on his own, and perhaps the hardest, not able to recognize his friends and family.  But I remind myself that he is comfortable, not in any pain, and unaware of his current state.  Sometimes I wonder why God has not taken him home yet, but most days I see him, I’m thankful, even though he doesn’t know who I am and he isn’t the man I once knew, he is there physically for me to visit and admire. </p>
<p>Recently, as I was clearing out his home preparing to rent it out, I came across some photos of him in his heyday.  Leaning back comfortably in a chair at a dinner party with friends, smiling at the camera, he seemed to be saying, “I made this life. I’m successful, I have many friends, I’m happy, and I’m going to enjoy this life to the fullest.” It made me, even more than before, want to emulate his success and character.  And seeing him, even in his current state, is a reminder to not let him down.</p>

<p>He currently has a nighttime caretaker, who makes sure he gets a good night sleep, then baths, dresses him and feeds him breakfast in the morning.</p>
<p>All the caretakers have been really nice and fun, and I think Gramps enjoys having the company.</p>
<p>Recently, his main caretaker, Ric, was getting Gramps dressed and singing Christmas carols as he did so. He asked Gramps,&#8221;What is your favorite song?&#8221;</p>
<p>Gramps replied &#8220;Oh How I Hate to Get Up in the Morning.&#8221;</p>
<p><strong>Alzheimer&#8217;s Fact:</strong> People with Alzheimer&#8217;s disease who also have diabetes or high blood pressure are more likely to die sooner than people without such disorders.</p>

			<content:encoded><![CDATA[<p><span style="color:#000000;">Things with Gramps haven&#8217;t been so funny lately.  As his Alzheimer&#8217;s progresses, it gets a little bit more difficult to smile through the stress of making decisions for his care, such as which doctors to go to, in house care or assisted living, etc.  However, once in a while, a little bit of the funny guy he once was shines through.  Such was the case yesterday.</span></p>
<p><span style="color:#000000;">I had to run an errand for a client, so I took Gramps with me.  It required driving through a city that is notoriously foggy, and yesterday was no exception.  </span></p>
<p><span style="color:#000000;">As always, Gramps was reading any lettering that we passed: street signs, store signs, the makes of cars on the trunks.  I&#8217;m not even sure how he could make out half the signs for the fog, since I could barely see where I was going.  </span></p>
<p><span style="color:#000000;">One particular sign on a brand new apartment building read &#8220;Million Dollar Views.&#8221;  Gramps read the sign, then looked toward the foggy view.  &#8220;Hmmph,&#8221; he said, &#8220;Not today!&#8221;  </span></p>
<p><span style="color:#000000;"><strong>Alzheimer&#8217;s Fact:</strong> The financial cost of caring for someone with Alzheimer&#8217;s disease can be overwhelming and is estimated to be about $50,000 per year in direct medical expenses. </span></p>

			<content:encoded><![CDATA[<p><span style="color:#00ccff;"><span style="font-size:7.5pt;font-family:Verdana;"><span style="font-size:10pt;color:black;font-family:Arial;">Gramps has been pretty un-entertaining lately.  Though don&#8217;t think he isn&#8217;t keeping us on his toes.  His Alzheimer&#8217;s is progressing, so most of the situations aren&#8217;t so funny anymore.  But just when my head starts to droop, leave it to Gramps to get me chuckling again! </span><span style="font-size:10pt;color:black;font-family:Arial;">Yesterday as we were finishing lunch, a fly landed on Gramps&#8217; empty plate.  Normally, he would just shoo the fly away but yesterday, he decided to take a more innovative approach to pest control. </span></p>
<p><span style="font-size:10pt;color:black;font-family:Arial;">He took his nearly full cup of coffee and poured it on his plate trying to douse the fly.  </span></p>
<p><span style="font-size:10pt;color:black;font-family:Arial;">When I screamed, &#8220;Gramps! You shouldn&#8217;t do that!&#8221;  he just looked at me with the mischievous smile of a two year-old, who knew he had done something wrong, but couldn&#8217;t figure out quite what. </span></p>
<p><span style="font-size:10pt;color:black;font-family:Arial;">To make matters worse, at that precise time, one of our clients walked into the restaurant and over to our table.<span>  </span>My heart sank as she glanced disapprovingly at Gramps’ plate.<span>  </span>She must have been wondering “How did his coffee get on his plate?”<span>  </span>I am sure “by means of extermination” was not a thought that crossed her mind. </span></p>

			<content:encoded><![CDATA[<p>This weekend while I was away, my mom watched my dog.  Being that my mom lives with my grandfather, he got some puppy time too.</p>
<p>As always, I prepackaged Jackson&#8217;s food into individual servings to make it easier for my mom.  My mom had placed the food on top of the refrigerator for convenience, but when she came home one day to feed the dog, the food was no where in sight. </p>
<p>After searching high and low, she finally opened the freezer and found the food in there.  Gramps must have been trying to do his part to keep Jackson cool during the hot summer days, and thought the food would be better slightly chilled. </p>
<p><strong>Alzheimer&#8217;s Fact:</strong> In addition to looking for a cure, researchers are focusing more and more on supporting the caregivers who spend upwards of 13 hours a day caring for loved ones.</p>

			<content:encoded><![CDATA[<p>One of the things Gramps still enjoys doing is gambling.  In the past, his games have been poker and craps.  As of late, he has been sticking to Paigow and the slot machines, which are a little bit easier for him to understand these days.</p>
<p>A couple days ago, my mom took Gramps to an Indian casino and set him in front of a slot machine.  She had him betting the minimum credits to draw out the game.  However, on one particular pull, Gramps bet the maximum number of credits without understanding what he was doing.  And wouldn&#8217;t you know it on that particular pull, he HIT THE JACKPOT!  It was only a penny machine, so he got himself 3,000 shiny new pennies, but what luck.</p>
<p>This luck might be explained by Gramps rather unique method of consuming fortune cookies.  </p>
<p>Instead of reading what his fortune holds, lately, he has been forgetting that there is, in fact, a fortune in fortune cookies, so he consumes the cookie whole&#8230;fortune and all!  Perhaps this is an ancient Chinese secret that they forget to give you with the bill for your chow mein: To make your fortune come true, eat it!</p>
<p><strong>Alzheimer&#8217;s Fact</strong>: The FDA has approved five drugs to be used in Alzheimer&#8217;s treatment: memantine (Namenda), tacrine (Cognex), donepezil (Aricept), rivastigmine (Exelon), galantamine (Reminyl).  About half of the people taking these medicaitons show modest and temporary improvement in memory and thinking skills.</p>

<p>In the last couple weeks, Gramps has been pulling half dollars out of his pockets.  My mother couldn&#8217;t figure out where he was finding him, but since he has quite an extensive coin collection, she figured he had a secret stash some where and didn&#8217;t think much of it.</p>
<p>However, my mom came home yesterday afternoon to find Gramps digging through a jar of coins in HER closet and removing all the half dollars and putting them in his pocket. </p>
<p><strong>Alzheimer&#8217;s Fact</strong>: Alzheimer&#8217;s disease is a progressive disease which advances in stages from mild forgetfulness and cognitive impairment to wide spread loss of mental abilities and total dependence on a caregiver.</p>

<p><span style="font-size:11pt;color:#000000;font-family:Arial;">&#8220;Gramps that wasn&#8217;t your money,&#8221; I said. &#8220;That was for the cleaning lady.&#8221;  </span></p>
<p><span style="font-size:11pt;color:#000000;font-family:Arial;">&#8220;I know,&#8221; he said. &#8220;But we shouldn&#8217;t be paying her that much.  I&#8217;ll clean the house.&#8221;  Evidently the fact that the housekeeper was already hard at work earning her pay when he made this decision escaped him. </span></p>
<p><span style="font-size:11pt;color:#000000;font-family:Arial;">Anyway, I told Gramps that he had to give the money back to my mom that night.  He agreed.</span></p>
<p><span style="font-size:11pt;color:#000000;font-family:Arial;">At lunch time, Gramps tried to buy me lunch with the $100 bill.  &#8220;That&#8217;s not yours,&#8221; I explained to him.  </span></p>
<p><span style="font-size:11pt;color:#000000;font-family:Arial;">&#8220;I know,&#8221; he said.  Evidently the fact that he shouldn&#8217;t be spending money he knew wasn&#8217;t his, escaped him as well. </span></p>
<p><span style="font-size:11pt;color:#000000;font-family:Arial;">At home that night, my mom asked Gramps for the $100.  He in turn gave her a one-dollar bill.  </span></p>
<p><span style="font-size:11pt;color:#000000;font-family:Arial;">Sometimes I wonder if this Alzheimer&#8217;s thing is real, or if Gramps just found a way to make a quick buck. </span></p>
<p><strong><span style="font-size:11pt;color:#000000;font-family:Arial;">Alzheimer&#8217;s Fact:</span></strong><span style="font-size:11pt;color:#000000;font-family:Arial;"> Communicating with Alzheimer’s Patients&#8211;Simplify each task into single steps. Use simple words and short sentences to convey your message. Do not overwhelm the person with a string of instructions. If a person has difficulty hearing, lower the pitch of your voice rather than increasing the volume.</span></p>

		<title>Not Those Damn Teeth Again</title>

			<content:encoded><![CDATA[<p><span style="font-size:10pt;color:black;font-family:Arial;">Gramps went to the dentist again yesterday.  And I&#8217;m just resigning myself to the fact that dentist = adventure. </span></p>
<p><span style="font-size:10pt;color:black;font-family:Arial;">In spite of his morning protests claiming nothing is wrong with his teeth and he doesn&#8217;t have to go to the dentist, I was able to get him to go to the dentist by explaining that he was going to have his teeth cleaned, and that this was to prevent any problems in the future.  </span></p>
<p><span style="font-size:10pt;color:black;font-family:Arial;">When Gramps got back from the dentist, he was very proud of his pearly whites and smiled to show me.  &#8220;Nice,&#8221; I said.  </span></p>
<p><span style="font-size:10pt;color:black;font-family:Arial;">Obviously Gramps was looking for a more enthusiastic response from me, and proceeded to take out his teeth to show me how clean they were.  </span></p>
<p><span style="font-size:10pt;color:black;font-family:Arial;">&#8220;See, see?&#8221;  he kept saying.  </span></p>
<p><span style="font-size:10pt;color:black;font-family:Arial;">Not wanting to see the teeth, I kept my head down and focused on my work.  Gramps was persistent though, and every couple minutes would take his teeth back out.  &#8220;See?&#8221;  </span></p>
<p><span style="font-size:10pt;color:black;font-family:Arial;">Finally I had enough.  &#8220;Gramps, you need to keep your teeth in your mouth.&#8221;  </span></p>
<p><span style="font-size:10pt;color:black;font-family:Arial;">&#8220;Just look,&#8221; he said.  &#8220;Can you take YOUR teeth out?&#8221; he asked, as if this was some miraculous super-human trick he had perfected.  </span></p>
<p><span style="font-size:10pt;color:black;font-family:Arial;">&#8220;No, Gramps.  And you should keep your teeth in your mouth.  They are disgusting.&#8221;  At this point I hear giggles coming from the offices of our tenants, who until this time had been able to keep quiet during our dialogue.  </span></p>

		<title>Over Dressed for Breakfast</title>

<p>First off, my mom thought she would be nice and make Gramps eggs benedict for breakfast. </p>
<p>As she started, he instructed her that she was doing it all wrong. </p>
<p>He then went to the cupboard and pulled out a box of Cheerios.  &#8220;See?&#8221; he said. </p>
<p>Then as he was eating his gourmet breakfast (the eggs benedict, not the Cheerios), he noticed his pants were rather poofy, and commented that he couldn&#8217;t find his belt. </p>
<p>My mom went to investigate his clothing and found that he was wearing TWO pairs of pants.  And the reason he couldn&#8217;t find his belt, was that he was wearing it on the first pair, which was underneath the second pair! </p>
<p><strong>Alzheimer&#8217;s Fact:</strong> High cholesterol and diets high in saturated fats increase the risk of Alzheimer&#8217;s disease, while diets high in antioxidants decrease the risk.</p>
